Good advice highlights all the advantages and disadvantages of an operation – and thus helps you make the right decision.

Size: A 3D scan can help to show the breast implant size on your own body. You can also "make your own test implants" (cook rice and insert it under a sports bra using a flexible PVC bag) - this gives you a good feel for the correct and desired weight and also feels the difference in terms of gravity.

With which material Should the breast be enlarged?

• autologous fat is only a real alternative in some cases: even if the fatty tissue heals perfectly, the realistic volume that can be achieved is usually a maximum of 1/2 cup - even with optimal results, it is usually not possible to increase the size any more than that - so it is usually not the method of choice for an average desire for more than 1 1/2 cup. For 1 1/2 cup, 3 to 4 or more procedures would be necessary with the same effort and at least the same cost per procedure compared to breast enlargement with implants.

Where should the cut = scar best placed?

Incision = scar. This applies to all operations. In principle, the shorter the incision, the better. However, a longer incision can also result in a better shape. So it's important to find a suitable compromise.

Before surgery, scars and their length are of course always an issue. After surgery, I have usually found that a shape that is as perfect as possible seems more important than saving scar length at all costs. A further improvement of the shape through a small tightening procedure (usually possible under local anesthesia) is of course also possible at a later date.

The cut in or just above the inframammary fold is usually considered the gold standard and is used very frequently. Scar quality is generally very good in the area of the inframammary fold. – We also offer you the premium version with shorter cut A funnel (HydroCone at Perle implants, sleeve for Motiva implants,…) to insert the implant as gently as possible. Another common cut is the around the areola. It is particularly useful if you have a very large areola or if the nipples are in different positions. Or if you want the scar to be exactly around the nipple. A small bikini makes it easier to hide a scar.

An additional incision from the areola vertically downwards lifts the breast gland tissue in the lower half. In the upper half, the volume is not really noticeable in the long term (this is why the patient's own fat or an implant is often used to build up the volume).

Also a cut in the armpit is one option. The advantage is that there is no scar in the chest area. But the scar in the armpit area can be very difficult to hide - especially if it is not so attractive. The exact positioning of the implants can also be more difficult in some cases and must be taken into account.

implant above or below the pectoral muscle (or partly/partly, but also subfascial (under the muscle fascia))? There is no right or wrong here. Only advantages and disadvantages. If only because submuscular is not always handled in the same way. One advantage of under the muscle is usually that the transition in the upper area is more natural and less bulbous.

What should you have heard/read before breast enlargement?

capsular fibrosis: Like any foreign body, a breast implant causes a connective tissue capsule to form around the foreign body (breast implant). This means that 100% of patients with breast implants develop capsular fibrosis. This also has the purpose of protecting the implant and keeping it in position. A follow-up operation is only necessary for capsular fibrosis if it causes problems - distortions, pain, etc. - then the normal state can be restored with an operation in which the capsule is usually cut or partially removed and a new implant is almost always inserted.

You should avoid…

• implants that are too large (from a certain size onwards, the possibility of complications and the risk of capsular fibrosis simply increase, the tension on the scar becomes stronger and the scar is therefore usually of poorer quality, and the incision also naturally becomes larger as the size increases)
• short cuts at any price (after surgery, a good shape is always more important than the length of the incision)

Deadlines must be met in Austria (surgical information and anesthesia information) – this means that there is a waiting period of at least 3-4 weeks from the decision to undergo breast enlargement, but often longer.

And there is every reason to think positively, because breast enlargement is (if everything is well planned, the team works well together and the surgeon is experienced) a very predictable procedure. But of course no surgeon in the world can completely rule out risks.
