click on the left E-mail address
choose "Patient" out of
Enter the required Data a
Read the Terms and Conditions and the Privacy Policy, check the boxto indicate that you have read and accept them and then click "Register"
Enter the required medical data And click on "Further"
Click on "Register your implants"
Enter the required data about your implant(s) by clicking on the Use Motiva ID card numberthat you received from us after the operation
Select your registration type:
- Option A: serial number (SN)
- Option B: Electronic Serial Number (ESN) – only for implants with Q Inside safety technology
Enter your SN or ESN and the online validation code which you will find on the right side of the traceability label
Click on "Send"to register your implants. You will receive a confirmation email. which confirms that your implant(s) has been successfully registered, as well as a second email with instructions on how to create your password
All Motiva implants are Always Confident Warranty for the entire life of the device and by product replacement insurance against Baker Stage III and IV capsular contracture for a period of 10 years.
