What is Preservation Rhinoplasty?

Subtle changes can have a big impact. And in many cases they are also more natural.

This is the principle of Preservation Rhinoplasty. The aim is to preserve the structures (structural preservation rhino) and especially the nasal bridge/roof (nasal bridge-preserving rhinoplasty=dorsal preservation rhinoplasty) as best as possible. The aim is to achieve the best possible result with maximum protection of the structures and skin and soft tissue on the nose. The anatomy of the nose is thus respected as much as possible.

The idea is not new. In recent years, it has been increasingly used and promoted by some nose surgeons. Some improvements have led to a real comeback of preservation rhinoplasty.

This does not involve removing a hump in the classic way or reducing the height of the nose with a chisel. Rather, the height is reduced by removing parts of the side walls and the nasal septum. This means that the bridge of the nose and many important nasal structures are retained in their basic shape.

One of the most important goals of aesthetic rhinoplasty is to ensure that there are as few irregularities as possible. The more changes are necessary (but also carried out), the higher the risk that irregularities may occur.

And this despite the most careful reconstruction! Scarring and twisting of the skin/soft tissue and also the cartilage lead to changes in the surface of the bridge of the nose in the first one to two years.

Push-down or let-down technique

Push-down or let-down technique...what does that mean?

A cartilaginous-bony strip is carefully removed from the nasal septum directly below the bridge of the nose. The entire bony nasal pyramid is then separated from the facial plane, allowing it to move and relocate to the facial plane ("Push-Down")

Depending on the severity of the nasal hump, a bony strip can also be removed from the sides of the nasal pyramid. The resulting gap can be used to reduce the nasal projection and move the nose even more into the plane of the face ("Let-Down").

An ultrasonic scalpel (piezoelectric instrument) is often used to mobilize the bone. The precision is significantly higher and the risk of unwanted bone fractures can thus be prevented.


When can Preservation Rhinoplasty be used?

Whether this technique is the right one for your specific case will be clarified in a detailed consultation. Depending on the surgical plan, parts of the surgical technique may not be decided until the operation itself.

In our experience, the technique is well suited for patients whose nasal bridge is straight and laterally symmetrical, or for patients with a crooked nose that is straight in its axis. 

A nose with a so-called S-shape (looks like the letter S) is usually suitable for this type of rhinoplasty.

Patients with very thin skin benefit in particular from the protection of the structures and the bridge of the nose. Scarring/twisting of cartilage and soft tissue is particularly visible through the skin when the skin is thin.

In our experience, the technique is particularly suitable for smaller humps where only minimal changes are desired and the emphasis is on naturalness.

If you want a “classic Barbie nose” with curves, conventional techniques may work better.

Every nose is different. Therefore, the concept is usually individual. A good surgeon will analyze your wishes as precisely as possible, discuss them with you and select suitable methods. He will also tell you what the possible limitations and risks of the planned operation are.


What are the most common risks?

Despite impressive advertising by some surgeons on social media – especially abroad, where a “magic finger” is enough to transform a hump nose into a beautiful one in an instant – this technique (like any other) carries certain risks. 

In some patients, despite all measures, the hump may become visible again during the healing process. correction In such (rare!) cases, this is generally possible without any problems after about one year.

Other patients may experience a palpable and possibly visible step on the upper part of the bridge of the nose (in the area of the root of the nose). This can be remedied with simple masking measures using the body's own cartilage.

What anesthesia is used for preservation rhinoplasty?

This is decided in consultation with the surgeon and anesthetist. In our opinion, the preferred option is gentle general anesthesiaWith this form of anesthesia, the airways are secured. Patients are not aware of anything and the surgeon can work undisturbed.

The type of anesthesia is part of the explanation and will be discussed with you in detail. In the case of gentle general anesthesia, the anesthetist will provide separate explanations.


Do I have to stay overnight for my Preservation Rhinoplasty in Klagenfurt Carinthia?

The nose surgery in Klagenfurt is carried out in a private operation (aesthetic operation). You should take it easy for 2-3 weeks. Do this with sensitivity and common sense. The basic rule is: do not lift more than 5kg, do not make sudden movements, do not work overhead.

But you are not sick. That means you should move around, but just take care of the operated area. When lying down, turning to the left and right is allowed and advisable.
